4. Audio and Lyric Sheet of New Song
5. Is the Spirit song the only sample? If not, please note other sample(s)
6. Name of Recording Artist
7. Record Company Name
8. Name of Distributor
9. Complete Contact Details (Name / Address / Tel / Fax / E-mail)
